Hearing loss due to noise has been described as primarily a condition of modern society. [35] In preindustrial times, humans had far less exposure to loud sounds. Studies of primitive peoples indicate that much of what has been attributed to age-related hearing loss may be long term cumulative damage from all sources, especially noise. People living in preindustrial societies have considerably less hearing loss than similar populations living in modern society. Among primitive people who have migrated into modern society, hearing loss is proportional to the number of years spent in modern society. [36] [37] [38] Military service in World War II , the Korean War , and the Vietnam War , has likely also caused hearing loss in large numbers of men from those generations, though proving that hearing loss was a direct result of military service is problematic without entry and exit audiograms. [39]

Clinically, the success rate of Intratympanic steroid therapy in patients with SHL is variable in the literature and the available studies are limited to retrospective and non-controlled prospective ones. In those studies steroids were used in various concentrations, regimens and delivery methods and their effectiveness have not been established due to the lack of randomized controlled trials. There have been some studies in the literature that discussed the effectiveness of Intratympanic steroid therapy as a salvage mode of therapy in patients who failed to respond to oral steroids (Herr & Marzo 2005, Slattery et al 2005).

• The animal's hearing may in fact have been diminishing over time, but the owner may have been unaware because the animal compensated until it reached a point where it could no longer hear adequately to get by. To the owner the onset appears sudden, but it has been progressive. This is common in older animals with presbycusis (age-related hearing loss), which is progressive with time and cannot be prevented or reversed. Because of the urgency for treatment, the employee’s primary physician was contacted and an appointment was set for that afternoon. He saw an otolaryngologist (ENT) the next day. The employee was given a 10-day course of prednisone and asked to avoid noise. In instances when he could not avoid noise on/off job, he was asked to wear hearing protection with a good seal. In discussing the case with the ENT physician, we discussed the dilemma of starting steroids when many times hearing will recover spontaneously. He explained that once he has ruled out any known causes, he discusses with the patient the possible steroid side effects and the chances of spontaneous recovery.
